Strip away the marketing and a 2026 companion robot does four things well. It holds an unscripted spoken conversation without a wake word for every turn. It remembers a routine — when you usually eat, when you take a walk, what you asked it yesterday — and surfaces it without being asked. It occupies space: footsteps, movement in peripheral vision, a presence in the room that is not a screen. And it notices when a routine breaks, which is the feature buyers describe as mattering most six months in.
What it does not do is a longer list. It does not provide clinical supervision. It does not reliably detect a fall. It does not replace human contact, and the owners who are still happy after six months are unanimous that they never expected it to. It is also not a general household worker — a companion-configured unit spends most of its day talking, following and waiting, not carrying things.
That gap between the two lists is why so much companion-robot search traffic ends in disappointment. The purchase works when the job is presence, conversation and routine for an adult who lives alone and would otherwise spend the day in silence. It fails when the job was really care.
Companionship is not care — check which one you are shopping for
This is the single most useful thing to settle before you spend anything, and it takes one question: if the robot did nothing but talk, keep a routine and be present, would that be a success?
If yes, you are shopping for companionship and this guide is the right one. If your honest answer involves medication reminders that must not be missed, fall detection, mobility assistance, or keeping a family member or care agency informed, you are shopping for assistive care. A care system that misses a dose is a hospital visit.
The distinction matters commercially too, because “companion robot” and “personal care robot” are marketed almost interchangeably and are not the same product. Plenty of units sold as companions have no clinical certification of any kind, no escalation path to a human, and no guaranteed uptime. Assume none of that exists unless the manufacturer states it explicitly in writing.
Age is not the dividing line either. A meaningful share of companion buyers are adults in their thirties and forties who work remotely and live alone, and their requirements — conversation quality, presence, privacy — look nothing like an eldercare specification. Ask to see what the robot does during an hour of nothing, and be honest about whether idle motion in your living room would comfort or unsettle you.

What you want is a unit that builds a model of a normal day over months and can notice a deviation from it.
